IBM Study: Eight in Ten EMEA Enterprises Escalate AI Incidents to Board or C-Suite as AI Agents Test Governance

IBM research finds 80% of EMEA executives saw an AI incident reach the board, but only 40% changed governance.
IBM surveyed more than 3,200 senior executives across eight countries in Europe and the Middle East, and eight in ten said an AI-related incident, such as an agent failure, data exposure, AI decision error or security breach, reached their board or C-suite in the past year.
Only 40 percent of those organizations changed AI governance policy in response.
When deployment outpaces governance readiness, 56 percent press ahead within agreed risk limits and about a third delay deployment.
