Your AI Agent Is Turning Exceptions Into Policy
Techstrong.ai, Friday, September 11th, 2026
AI agents that treat past one-time exceptions in operational records as precedent can make organizations consistently wrong at machine speed.
When an AI agent retrieves a record of a one-time approved exception, it may present it as company policy, even though every fact is correct. Organizations focus on what data agents can access but not on how agents interpret decisions found in operational history.
The author argues that being able to retrieve a decision does not give an agent authority to generalize it.
Context should be separated into policy, procedure, exception and anecdote, with only policy and current procedure reusable by default; exceptions should instead prompt questions for a human.
